项目摘要

本研究は、現在助産所での併設や助産師による新規開設が増加しつつあるハイリスク妊産婦または医療的ケア児を支援対象とした訪問看護ステーションで実施されている活動について、その実態および効果を明らかにするために訪問看護ステーション利用者と支援者である助産師の経験を聞き取り調査によって、明らかにすることを第1の目的としている。研究初年度の2021年に引き続き2022年度も聞き取り調査を続行し、利用者8名、助産師7名への調査を終了した。現在分析途中であるが、利用者からの聞き取り調査では、訪問看護の対象は、医療的ケアが必要な母親もしくは子どもではあるが、助産師は疾患等のケアだけではなく妊娠・出産および子育支援を含めたケアを提供していると対象者がとらえていることが明らかとなった。また、支援者である助産師は、ハイリスク妊産婦および乳幼児だからこそ、助産師がケアすることが必要であることを実践を通して感じていることが明らかになった。また、本研究は、支援者への研修プログラムを構築し、その効果を検証することを第2の研究目的としているが、そのプログラム作成のための基礎資料を得るために、訪問看護ステーション管理者と訪問看護ステーションと協働する専門職者（医師、保健師、理学療法士、ソーシャルワーカー）10名への聞き取り調査を実施した。その結果、訪問看護ステーション活動を行う助産師は、乳幼児の発達に関する知識や医療的ケア児への看護技術を向上させる必要があること、コミニュケーション能力の向上をさらに図る必要があること、母子へのリハビリテーションに関する知識を持つ必要性等が明らかになった。現在、この聞き取り調査の結果と日本訪問看護財団が提示し、訪問看護ステーション活動を実施する看護職の養成に活用されている「訪問看護人材養成基礎カリキュラム」をもとに支援者への研修プログラムを作成中である。

This study is aimed at increasing the number of new regulations for midwives in maternity clinics, including support for pregnant women, access to nursing care, and implementation of activities to clarify the results of visits to nursing care users and support midwives. The first goal is to make sure that you have a clear vision of the future. In the initial year of the study, 8 users and 7 midwives completed the survey in 2021 and 2022.
