项目摘要

本研究の目的は、幼児期の子どもを育てる家族が「子どもの緊急入院」という出来事に直面した時に、若手の看護師や、小児とその家族に接する機会が少ない看護師でも素早く、簡単に、的確に家族に起こった問題をアセスメントできるツールを作成することである。まず第１段階として「入院中の子どもをもつ家族の困り事に関する文献検討」をおこなった。その結果、子どもの入院に伴い、付き添いが余儀なくされ、生活の場が二分されることで、家族内での役割調整が生じたり、入院中の病院での生活での心身の負担や気苦労が発生していた。また、子どもが入院した家族は、支援を必要としているにもかかわらず、支援者として最も身近である祖父母にも遠慮や気遣いがあり、支援も求めにくい状況であることが明らかになった。しかし、「子どもの緊急入院」という出来事が起こった幼児期のすべての家族が支援を必要としていることではないことや、子どもの年齢や疾患の重症度や治療によっても異なること、家族員によって、子どもの入院という出来事に対する捉え方が異なることなどが明らかになってきた。さらに、文献検討を重ねていくなかで、子どもに健康問題が生じた時の対応は、親による子どもの健康管理能力が大きく影響することが明らかとなり、親の養育行動や家族の生活力量モデル等にも着目して文献検討を進めている。以上のことから、育児期家族への家族支援を検討していく中で、再度、研究の対象とする子どもや親の状況を（年齢や疾患、重症度などを）限定していく必要性が生じ、育児期家族の養育行動に着目して、さらなる文献検討を行い、研究計画の修正を進めている段階である。
