项目摘要

生活保護受給等の経済的生活困窮者・家族への健康自己管理支援のため、市町村の生活保護担当課に配置された保健師(以下、生活保護担当課保健師)による支援活動の特徴を明確にし、食育推進と生活習慣病予防等のセルフケア能力獲得に向けた支援活動方法を検討する。特に幼少・学童期からの糖尿病等生活習慣病予防自己管理に向けて、学校給食と子ども食堂実施者との協働について提言する。生活保護担当課保健師による生活保護受給者等の経済的生活困窮者・家族への食育推進・糖尿病等生活習慣病予防健康自己管理支援の方法とアウトリーチ機能について、熟練保健師(原則5年以上の実務経験)の実践事例を調査する。データ収集は、生活保護担当課保健師に個別支援再現記録を作成してもらい、研究者がその記述内容を理解するためインタビューで内容確認する。ここでの記述表現は、保健師が内面で考えたことが、事実に忠実に伝わるよう、研究協力保健師が文章推敲・加筆修正し、再び研究者が確認するというやりとりを合意できるまで繰り返す。データ分析は、保健師の行為と意図の内容に、視点をあてた詳細な分析を行う。分析の視点は、先行研究とこれまでの研究経過で修正を重ねて採用してきたもので、(1)看護援助を提供する者と受ける者との相互作用、(2)家庭・地域生活を含めた援助提供、(3)援助ニーズの優先度の判断と援助提供方法の選択、(4)対象の過去の経験に対する援助提供、(5)保健医療福祉サービスへの適用、(6) 看護援助の他事例や保健事業・施策への反映、(7)関係職種との連携、である。この記述データの記述と分析の適切さは、研究組織内の家庭訪問実践・教育経験のある看護研究者2名に検討してもらう。幼少・学童期からの糖尿病等生活習慣病予防自己管理に関して、子ども食堂実施者と健康生活指導に関わる小中学校教諭・養護教諭から食育推進・生活習慣改善への取り組み実態を調査する。
