本研究の目的は、特定行為研修を受講した訪問看護師が行う①特定行為実践の現状と効果及び実践を妨げている多面的な要因は何かを明らかにし、②問題解決に向けた特定行為在宅実践モデルを構築することである。①の目的について、2021年度のインタビュー対象者（特定看護師5名、医師1名）に加えて、2022年度は新たに4名の特定看護師にインタビューを実施した。分析の結果、2021年度の分析結果からは十分明らかになっていなかった特定行為実践のための阻害要因と促進要因があった。研究開始当初から医師との信頼関係を構築することへの課題があったが、2022年度のインタビューデータからも同様の結果を得た。その他には、在宅医療が充実してる地域では特定行為のニーズが無いことや、医師に手順書を作成してもらうためには組織が関与しなくては特定看護師のみの力では困難であることが明らかであった。特定行為が実践できている看護師は、その地域でどのような活動をしてるか分析を試みた。その結果、地域住民への啓蒙活動、さらに医師が集まる学会で、看護師が特定行為を実践することで利用者や地域に役立っていることを発表していた。また自ら進んで手順書作成にかかわり医師に提示するなど積極的にアプローチしていた。2023年度は明らかになっていなかった多面的な要因について学会発表する予定である。②に対しては、1名の対象者の実践が他の特定看護師のモデルとなると考え、事例を分析した。分析結果から、やはり医師との関係構築が重要で、医師にアプローチする方法を考えていた。特定行為の運営規定やマニュアルを作成し実践していた。特定看護師でありながらも特定行為を実践できていない看護師にとって参考になると考える。この事例を事例研究として国際老年看護学会で発表予定である。
