本研究では、在宅で介護をしている介護者の社会的孤立を予防するために、在宅ケア従事者のための「社会的孤立予防プログラム」を開発することを目的としている。2022年度実績は、以下のとおりである。研究目的：介護者の社会的孤立予防において効果的な保健師の支援内容を明らかにし、「社会的孤立予防プログラム」を作成する。研究方法：Ａ市の地域包括支援センターに所属している保健師で、過去3年以内に介護者の社会的孤立予防に関わった経験を有する者を対象に、半構成的面接調査を実施した。調査内容は、保健師の基本属性、介護者（事例）の特性、事例への関わりのきっかけ、社会的孤立と認識した理由、関わりの経過、問題解決のために行った支援内容、支援による反応や効果であった。面接予定者のうち、5名の面接を実施した。対象者の了解を得て会話内容を録音し、逐語録を作成した後、社会的孤立予防に効果的な支援内容を質的記述的分析法により抽出した。結果：保健師は、社会的孤立を認識していない状態の介護者が必要な支援を受入れられるよう信頼関係を築きながら介入の糸口を見出していた。次いで、療養者及び介護者の健康状態、生活状況、社会との関わり、経済状況等の視点からアセスメントを行い、療養者の生命と生活を守るために必要な医療・介護サービスを導入していた。さらに、民生委員、医療機関、職場、認知症家族会、司法書士など、関係専門職種・関係機関との連携を図り、療養者と介護者が家庭や地域で安心して生活を送るためのネットワークづくりを行っていた。その結果、介護者のストレスが軽減、日常生活が整う、職業の継続、近隣住民との緊張緩和などの効果が生じていた。
