小児がんの治療成績は現在8割近くまで改善しているが、その多くが様々な晩期合併症を抱えながら社会生活を送っている。晩期合併症は、肥満、心疾患、呼吸器疾患、二次がん、認知機能障害など様々である。また、AYA世代のがん経験者は、治療しながら就労を続けるため、合併症や不調を抱えながら生活を送っている。このような就労期にある若い世代のがん経験者への支援として、Webを活用した健康維持プログラムが効果的であるといえる。本研究は、就労期の小児がん経験者とAYA世代のがん経験者に運動と栄養のWebプログラムに参加することで、晩期合併症への自己管理と予防・重症化予防ができるような健康習慣の獲得を目的としている。小児がんを対象とした運動と栄養のWebプログラムでは、疾患毎に発生頻度の高い晩期合併症を提示することで、予防効果を期待している。プログラムは、『健康情報』『運動プログラム』『栄養プログラム』『データ入力』の4部構成から成り立っている。令和3年度はFeasibility Study①として、専門職と小児がん経験者の計15名からプログラムに関するインタビュー調査を行い、プログラムの内容の適切性と実行可能性を評価した。その結果から、Webプログラムの内容を文字情報から映像に切り替え、理解のしやすさ、操作性を向上させた。令和4年度はFeasibility Study②として、6週間の短縮版のWebプログラムを4名の小児がん経験者に実施してもらい、適切性と実行可能性の評価を行った。その結果、就労者では時間が取れないこと、モチベーションの低下から、継続性の困難さが明らかとなった。そのため、フィードバックできる仕組みとレベル上げが可能なAIを用いたシステム変更が必要となり、現在、AI専門家との開発を進めている。
