本研究の目的は、夜間頻尿の高齢一般住民を対象に夜間頻尿特異的QOLとセルフマネジメントと下部尿路機能を調査し、その特徴と関係性を記述することである。2022年度はプレテスト、一般公募、データ収集を行う予定であった。しかし、COVID-19の感染拡大による社会的情勢により計画が進められず、文献検討、セルフマネジメント行動の調査項目の検討、倫理委員会申請準備を行った。文献検討は、「下部尿路症状」「セルフマネジメント」のキーワードで国内外含めて検索した。新たに下部尿路症状のセルフマネジメントに関する研究が国内外も含めて報告されており、国内では排尿日誌と合わせて認知行動療法のセルフチェックシート（起床時間・就寝時間、食事時間、昼寝、アルコール・カフェイン摂取、水分摂取、塩分摂取、運動、入浴の8項目）を連日記入した介入群の方が対照群より夜間頻尿に対する疾患特異的QOL質問票であるN-QOLの総スコアが有意に改善していたという報告があった。また、海外においても男性の下部尿路症状の治療におけるセルフマネジメント介入の有効性について、システマティックレビューで中程度の質のエビデンスが見出されたことが報告されていた。海外文献では、下部尿路症状のある男性に対する自己管理介入の主な構成要素は、（1）教育と再確認、（2）水分管理、（3）カフェインおよびアルコール摂取量の削減、（4）併用薬の使用、（5）トイレおよび膀胱訓練、（6）その他の介入に特定されていることがわかった。今年度の文献レビューで明らかになった構成要素からセルフマネジメント行動の調査項目を作成し、2023年度は、プレテスト、一般公募、データ収集を行っていく。

The purpose of this study was to investigate the characteristics and relationships of urinary tract function in the general population with high frequency urine at night. In 2022, the public offering and collection were scheduled. In view of the social situation caused by the increasing infection of COVID-19, plans are being made to carry out research, literature review, review of investigation projects of the "Operation Self-Management", and preparation of ethics committee applications. The literature review of "lower urinary tract symptoms" and "urinary tract symptoms" includes domestic and foreign information. New research on lower urinary tract symptoms, including urinary tract reports, and cognitive action therapy (8 items: wake up time, bedtime, eating time, daytime sleep, sleep time, water intake, exercise intake, bath time) The treatment of lower urinary tract symptoms in men is effective, and the quality of treatment is reported. Overseas literature shows that the main components of self-management intervention for men with lower urinary tract symptoms include: (1) education and reconfirmation;(2) water management;(3) reduction of urinary tract intake;(4) use of concomitant drugs;(5) bladder training; and (6) specific intervention. This year's literature review will focus on the development of research projects, the preparation of research projects for 2023, general public offerings, and the collection of research projects.
