项目摘要

令和4年度は、第二回目のフレイル健診の実施となり、新型コロナウイルス感染症対策を優先するなか、フレイル評価指標の測定、生活習慣のアンケート調査を行うことができた。測定地区は前年度と同じく35ヶ所76サークル、実施回数は88回、参加延べ人数は1136人であった。この内重複・測定未実施の278人を除く858人がフレイル健診受診者であった。今年度はWalkability Index（歩行環境得点）を入手し、フレイル健診受診者の住所位置情報と連結した。Walkability Indexはある地点から徒歩で到達できる範囲に、生活利便施設、商業・レジャー施設、教育・学び施設といった生活をする上で近くにあって利便性が高い「都市のアメニティ」がどれだけ集積しているかを最低50点、最高100点満点で評価する指標である。連結可能であった759人のデータから、日本語版フレイル基準による分類ではフレイルが 59人(7.8%)、プレフレイルが330人(43.5%)、非該当が370人(48.7%)であった。女性448人 (64.6%)、男性100人(61.0%)であった。フレイル、プレフレイル、非該当の 3群で比べると、前年と同じく、手段的ADLでは差を認めないが、フレイル群が最も高齢で歩行速度が遅く、情報収集能力や社会参加能力、そして伝達的・批判的ヘルスリテラシーが低かった。32の行政区別Walkability Indexの値では行政区の差を認めたが、3群間のWalkability Indexは差を認めなかった。二元配置分散分析によりフレイル、プレフレイル、非該当の 3群とWalkability Indexの交互作用が社会参加能力と有意に関連した。以上からフレイル分類とWalkability Indexの相互作用が、一部の高次活動能力に関連することが明らかになった。
