本研究は、自宅で生活する認知症高齢者が、疾患の進行や介護負担の増強により自宅での生活が限界を迎え、サービス付き高齢者向け住宅や認知症対応型共同生活介護（認知症高齢者グループホーム）等へ住まいを移す移行期に、そのケアに密接に関わる施設の介護職、ケアマネジャー、訪問看護師が実践している支援および連携の現状と課題を明確化し、認知症高齢者が新しい環境へ適応することを支援するためのプログラム開発の基礎的資料を得ることを目的としている。住まい移行期にある認知症高齢者に対する、訪問看護師、ケアマネジャー、介護職の支援内容および連携の現状と課題の明確化に向けて実施した国内外の文献レビューでは、住まいの移行期に着目した研究はほぼ見受けられなかったが、施設における認知症高齢者ケアに対する困難感を抱える介護職が多いこと、多職種からの支援体制を整える必要があることが示唆された。2022年度は、質問紙調査項目の選定のため、認知症高齢者が高齢者施設へ住まいを移行することが一般的である、北米やヨーロッパ、北欧を中心とした海外文献の再検討を実施した。住まい移行時の数か月間は、認知症高齢者のQOLが低下すること、新しい環境への適応には、看護師および多職種による支援が必要であることが示唆された。支援内容の具体的な方略についてはさらなる研究が必要であり、質問紙調査の項目の選定へと繋がった。次年度は、各専門職の支援内容、連携の現状や課題に関する質問紙調査、およびインタビュー調査の準備を実施する予定である。
