本研究ではリハビリテーション病棟に入院する脳卒中患者101名を対象に、Toileting Tasks Assessment Form（TTAF）を用いて、トイレ動作を構成する諸動作の自立度を評価し、諸動作のスキル再獲得の難易度を調査した。TTAFは、車椅子を利用している脳卒中患者がトイレ動作を利用する際に必要な動作を24の諸動作に細分化して、それぞれの自立度を評価するためのツールである。評価は担当の療法士が入院時からトイレ動作が自立するまで、または病院を退院するまで実施した。スキル再獲得の難易度は、諸動作において入院時に自立していない者のうち、評価終了までに自立に至った者の割合を算出し、割合が低いほど難易度が高く、割合が高いほど難易度が低いと定義した。本研究の結果、「おむつパッドの処理」が最も難易度が高かった。その他に難易度が高い項目は、「ナースコールを押す（排泄後）」、「フットレストから足を下ろす」、「清拭」、「ブレーキをかける」、「フットレストに足を乗せる」であった。入院時は車椅子・便座間の移乗動作や立位での下衣操作など、トイレの主となる動作の難易度が高かったが、スキルの再獲得においては、車椅子の操作や周辺機器・物品の操作など、周辺の動作の難易度が高いことが明らかとなった。今後は、患者の特性や能力に基づいたトイレ動作獲得におけるパターンを明らかにすることで、入院時から患者の自立のパターンを予測し、患者の能力と諸動作の難易度に基づいた練習計画の立案が可能となる。
