神経因性腸機能障害が脊髄損傷患者に及ぼす影響は多岐にわたり，生活の質の低下をも招くことから個々人に適切かつ快適な排便プログラムを提供する必要があり，神経因性腸機能障害治療のエビデンス蓄積が喫緊の課題である．本研究では，胆汁酸の持つ直腸知覚閾値の低下作用に着目し，神経因性腸機能障害を合併した脊髄損傷患者の直腸知覚閾値に対する胆汁酸および胆汁酸再吸収阻害薬（エロビキシバット）の効果を明らかにすることで，病態生理ならびに薬理作用に基づいた新たな治療選択肢や既存治療との組み合わせなど神経因性腸機能障害治療の幅を広げることを目的としている．上記目的達成のために研究期間内で行うことは以下である．１）脊髄損傷患者群および非脊髄損傷患者群における糞便中胆汁酸濃度測定２）脊髄損傷患者群におけるエロビキシバット投与前後の直腸肛門機能検査，糞便中胆汁酸濃度測定，神経因性腸機能障害重症度評価３）測定した胆汁酸濃度測定を用いた直腸肛門機能検査における３×３二重盲検クロスオーバー試験本研究では神経学的損傷高位が第５胸髄以上の不全脊髄損傷を有する６５歳未満の成人男性を対象としており，脊髄損傷患者ならびに非脊髄損傷患者（腸機能健常者）はそれぞれ１２名ずつの登録を予定している．当該年度では前年度に引き続き患者登録を継続したが，前年度と同様に新型コロナウィルス感染症流行に伴う入院患者数の制限等により患者登録に難渋した．脊髄損傷患者３名・非脊髄損傷患者３名が登録されたが，脊髄損傷患者３名はいずれも途中脱落となった（１名：経過中に完全損傷に移行，２名：エロビキシバット継続困難）．現在は２名が登録され試験継続中である．
