项目摘要

多血小板血漿（PRP)療法と体外衝撃波治療（ESWT)の両併用治療による有効性についての報告はほぼ認めない。ESWTでは組織修復作用と除痛効果、自由神経終末の破壊、血管新生を期待し損傷部に実施後、さらにPRP療法により組織分化増殖を促進することで相乗効果を期待できると仮説をたてた。、PRP＋ESWTの併用による難治性膝蓋腱炎に対する治療効果が明らかとなり、確実な早期スポーツ復帰に向けて効果的な保存療法の選択肢として認識されることである。そこで、本研究では、スポーツ選手の難治性膝蓋腱炎に対して、①PRP群、②ESWT群、③PRP+ESWT群に分けて患者立脚型機能評価質問票にて疼痛の変化を把握することとMRIや超音波診断装置を用いて画像評価を行うことからみえてくるPRPと相乗効果について比較検討試験である。現在、各群の参加人数が2-3人ずつである。全参加者は10名となっている。現段階で得られたデータを解析中である。①～③群全てでおいて治療効果を表す患者立脚型機能評価質問票では、症状の改善が認められた。そのデータから初回投与前と経過の症状やスコアの改善率を評価すると、③群において、他の群と比較して最も早期に症状が改善し、そして最も長く改善率が持続したことが6ヶ月間の評価においいて明らかとなった。統計学的な有意差はまだ認められていないが、今後参加人数が増加することで明らかになると考えられる。また画像評価については、まだ解析途中である。
