近年、糖尿病診療においてPHR（Personal Health Record）を用いたセルフモニタリングが推奨されている。これまでにスマートスピーカーやモバイル端末を経由して支援メッセージを送信する臨床用PHRは、日本では臨床応用されていない。本研究の目的は、既開発の臨床用PHR「電子糖尿病ダイアリー」に2型糖尿病患者のセルフモニタリングを励行する自動支援メッセージ機能を追加実装し、その有用性を検証することにした。令和3年度から4年度にかけて、徳島大学病院を通院しセルフモニタリングが必要な2型糖尿病患者を対象に介入研究を行った。対照群は、電子カルテ内の検査結果や処方情報を連携できるスマートフォンアプリケーション電子糖尿病ダイアリーを使用した（PHR群）。一方、PHR＋メッセージ群は、電子糖尿病ダイアリーにセルフモニタリングを励行する支援メッセージの自動送信システムを新たに実装した。令和3年度は、PHR群（n=10）とPHR＋メッセージ群（n=11）を対象に、研究開始から中間評価まで実施した。また研究開始から中間評価までの介入期間は、PHR群は99±21日、PHR＋メッセージ群は107±36日であった。令和4年度は、介入期間中に離脱者2名を認めた。その結果、PHR群（n=9）とPHR＋メッセージ群（n=10）を対象に、研究終了時の評価を実施した。また研究開始から終了時までの介入期間は、PHR群は201±45日、PHR＋メッセージ群は204±44日であった。介入終了時、両群間の各自己測定率やHbA1c、収縮期血圧の変化に有意な違いを認めなかった。一方、介入終了時のBMI変化量はPHR+メッセージ群の方が有意に低値であった。支援メッセージ機能を追加した電子糖尿病ダイアリーは、2型糖尿病患者のセルフモニタリングを改善しなかったが、体重管理の有効性が得られた。
