项目摘要

AYA世代にも発症する希少腎がんのなかで, 原因遺伝子と疾患概念が確立し, 生殖系列病的バリアント情報集積が進みつつあるBirt-Hogg- Dube(BHD)症候群と遺伝性平滑筋腫症-腎細胞がん症候群(HLRCC)を中心に分子病理学的研究を行った.HLRCC関連腎がん(HLRCC-RCC) は高悪性度で極めて予後不良だが, 医療関係者間の周知度が低く, 散発性type2乳頭状腎細胞癌との形態学的鑑別が困難で, 本邦の疫学研究はまだ殆どない. 全国医療機関の泌尿器科医, 病理医らと共同研究を行い, 10家系10名から切除されたHLRCC-RCC13検体について, 罹患者の病的バリアントや腫瘍の臨床病理学情報をまとめた. 海外では報告されていない病的バリアントを有し, 全て異なるパターンであった. うち2例でコピー数解析も行ったところ, 原因遺伝子FHのある1q43をカバーするように1番染色体長腕の広範囲で片親性ダイソミーが起こっていた. 更に2番染色体短腕の増幅も共通していた. これらの結果を国際誌に発表した.HLRCC-RCCに有効な治療法に関する情報はまだ殆どないが, 1名の進行癌患者に免疫チェックポイント阻害剤を投与し, 31週後に完全寛解を得たため, 診療や病理学的所見の詳細について報告した.BHD症候群では三主徴である肺嚢胞, 腎細胞癌, 皮膚腫瘍以外にも様々な腫瘍や代謝性疾患やとの関連が示唆される. 代表者らは自験例で脳出血や血気胸など出血性疾患が低頻度あることからFLCN機能と脈管形成との関連を想定していた. 今回モデル動物を用いた共同研究に参加し, 肺におけるリンパ管内の血液流入を同定し, 静脈(V)とリンパ管(L)の分化異常につながるデータを得た.肺嚢胞発症とL-V mixingの関連について今後更なる解明が期待される.

AYA Generation-Related Diseases: Birt-Hogg-Dube (BHD) Syndrome: HLRCC (Kidney Cell Syndrome): A Study of Molecular Pathology.(HLRCC-RCC) is highly pathogenic, poorly known among medical associates, and difficult to differentiate from sporadic type 2 papillary renal cell carcinoma morphologically. A joint study of urology and pathology in medical institutions across the country was carried out. Ten patients from 10 families were resected from HLRCC-RCC. 13 patients were diagnosed with HLRCC-RCC. Overseas, we report that there is a wide range of diseases in the country. 2 cases of chromosome number analysis, the cause of FH and 1 q43, 1 chromosome long arm and 1 chromosome long arm and 1 chromosome long In addition, the two chromosomes have a short arm and an increased amplitude. The results were reported in international journals. HLRCC-RCC had information on effective therapy, 1 patient with advanced cancer had immunologic therapy, and 31 weeks later, complete remission was obtained, diagnosis and pathological findings were reported in detail. BHD syndrome had three main features: lung cancer, renal cell carcinoma, and renal cell carcinoma. Other than skin swelling, swelling and metabolic disorders are associated with these disorders. The representative thought about the low frequency of bleeding, chest bleeding and vascular disease. In this paper, we study the blood inflow in the lung and the differentiation of the vein (V) and the tube (L). The relationship between lung cancer and L-V mixing is expected to be clarified in the future.
