大腸がん肝転移の根治的治療は外科的肝切除術であるが、肝切除が可能な症例が限られ、その場合は標準的治療として 全身化学療法が施行されるが、その治療に不応・不耐となった場合、予後は半年程度と極めて不良となる。そのような患者に対しては欧米ではイリノテカンを吸着させた球状塞栓物質（Drug eluting-bead loading irinotecan）による肝動脈化学塞栓療法（以下DEBIRI）が施行されているが、本邦では肝動脈化学塞栓療法自体は実施可能ではあるものの、イリノテカンの動脈内投与が適応外使用（静脈内投与は適応）であり、標準治療抵抗性の大腸がん肝転移の患者に対して、DEBIRIを実施することができない。本研究はそのような他に有効な治療法がない患者に対して、、主評価項目として、肝無増悪生存期間、副次評価項目として、全生存期間、無増悪生存期間、奏効割合、有害事象を検討し、DEBIRIの有効性、安全性を評価するものである。本年度は新たに6症例の登録があり、DEBIRIが行われた。grade3以上の有害事象の報告も数件あったものの、通常の肝動脈化学塞栓療法と同様あるいは本治療との因果関係の低いもので、研究を中止するような重大なものは認めなかった。症例登録は順調で、当初の予定の2022年7月31日までにすべての登録が完了した。また、症例の経過観察期間も終了し、2023年1月には画像の中央判定も実施した。

The treatment of radical liver transplantation is surgical hepatectomy. The treatment of standard systemic chemotherapy is not tolerated. The treatment of six months is extremely bad. A spherical plug substance is adsorbed on a patient's body. Drug eluting-bead loading irinotecan (DEBIRI) is administered to patients with liver metastases resistant to standard therapy. This study includes patient response, primary evaluation, liver free survival, secondary evaluation, overall survival, liver free survival, adverse event response, effectiveness and safety evaluation of DEBIRI. This year, 6 new cases were registered. There have been several reports of harmful events above grade3, as well as common hepatic arterial chemoembolization therapy. However, the causal relationship between this treatment and the study is too low, and the research is suspended. The registration of the case was completed on July 31, 2022. In January 2023, the central judgment of the case was completed.
