左心不全患者において、右室機能が予後を左右する重要な因子であることが報告されている。これらの報告では様々な手法を用い、右室機能評価を行っている が、非侵襲的右室機能評価指標が確立されているとはいえない。その理由の一つとして、右室の複雑な解剖学的形態があげられる。現在、心エコー法において、 新世代の経胸壁三次元心エコー法が登場し、リアルタイムに心臓の立体画像描出が可能となった。本研究の目的は、この三次元心エコー法を用いて右室の解剖学 的形態を評価し、右室容積を求め、我々が注目した右室機能指標(Estimated RV-Ea＝(三尖弁逆流最大圧較差＋下大静脈より推定する右房圧)÷(右室拡張末期容積 ―右室収縮末期容積))を算出し、この指標が、重症心不全患者の予後に与える影響を明らかにすることである。さらに、本指標と今まで予後と関連していること が報告されている指標、右室機能指標として報告されている指標との比較も行うとともに、右室心筋の線維化等の組織学的状態、運動耐容能との関係性を評価に 加える予定である。上記の研究を進めるにあたり、本年は、経胸壁三次元心エコー法の施行可能な最新の心エコー機を用いて、入院患者の内、対象基準に合致する患者について、三次元心エコー法を的確に行い、データ収集を行っている。結果として、43名の患者のデータの採取を行った。現在、引き続き、患者データを収集するとともに、予後の追跡を行っているところである。
