手術ハイリスク僧帽弁閉鎖不全症（mitral regurgitation; MR）患者に対する経カテーテル僧帽弁形成術マイトラクリップが保険適応となった。特に心不全に合併する二次性MRに治療され、薬剤治療に比較して心不全入院および全死亡を抑制することが海外の先行研究（COAPT試験）で実証された。近年心不全治療の一環として臨床応用されている6000例を超える症例で治療が行われている。本研究の目的は、二次性僧帽弁閉鎖不全に対する経皮的カテーテル僧帽弁修復術マイトラクリップの費用対効果を明らかにすることである。この研究と同様の研究成果はCOAPT試験対象患者からも発表され、薬物療法に比較してマイトラクリップ治療が費用対効果をみとめたとの結果が得られている(Circulation. 2019;140:1881-1891)。当初は当院と京都府立医科大学で前向きに患者登録を行う予定であったが、心不全に伴う二次性僧帽弁閉鎖不全となると介入後の費用をどのように算出するかが課題であった。現時点では多施設で費用を前向きに厳密に取得する手段が構築できていない状況である。施設間の費用算出が困難な状況である反面、本研究の目的である費用対効果には費用部分が極めて重要である。現在、実際の費用を算出できるか各施設で審査中である。また前向きに登録には患者をフォロー期間に余裕がないため、既存の治療後患者を対象とした後ろ向き研究に変更している。上記理由から研究に大幅な遅れを生じている。
