手指・手・肘関節を含む上肢の外傷・変性疾患に対して手術を行った場合、術後の上肢機能を最大限回復・改善させることが非常に重要である。しかし、近年の手術手技・固定材料の進歩をもってしても、術後の上肢の機能回復が十分得られず、日常生活動作が制限され、患者のADL・QOLが著しく低下することは少なくない。本研究の目的は、これまでに研究代表者らが研究を進めてきた炭酸ガス経皮吸収量療法を、手指・手・肘関節の手術後の上肢に適用することで、手術後の上肢の筋力回復の促進・関節可動域改善・疼痛改善の効果が得られるかを実際の臨床において検証することである。具体的には、①本邦で現在約1280万人と推計される骨粗鬆症患者において発生リスクが高いとされている橈骨遠位端骨折に対する内固定手術を施行した患者、②肘の代表的な骨折である肘頭骨折に対する内固定術を施行した患者、そして③変形性手指関節症に対する人工手指関節置換術を施行した患者を対象とした。しかし、2020年2月から流行し始めた新型コロナウイルス感染症の影響で、本臨床試験の参加者を募ることは困難を極めた。患者は本臨床研究に参加することにより、頻回の外来受診が必須となり、かつ研究者が被検者の上肢にハイドロゲルを徒手的に塗り込む必要性があることと専用アダプターを上肢に取り付け100%の炭酸ガスをアダプター内に送気する際に安全上の配慮から被検者の傍で研究者が異常の有無を観察しなければいけないことから、濃厚接触を余儀なくされる。そのため、新型コロナウイルス感染症の感染症法上の位置づけが5類感染症に変更される令和5年5月8日を待って、本臨床研究参加患者募集を開始することとした。
