项目摘要

①1996年に登録年数10年以内で06年まで追跡した医師72,477名（コホ―トA)と、2006年に登録年数10年以内で16年まで追跡した医師68,229名（コホートB）の2医師コホートの比較を行った。なお、1996年から2006年は医師養成数が減少しており、コホートBにおいてAよりも医師数は少ない。②コホートAにおける女性医師の割合は16.4%、一方コホートBにおけるそれは25.9%であり、コホートBにおいて9.5%増加していた。コホートAにおける平均年齢は31.9歳、Bでは31.6歳でありほぼ同じであった。③コホートAの1996と2006年における勤務市町村の人口の中央値は277.５、2942千人であった。都道府県庁地点からの勤務市町村市役所・役場所在地までの距離の中央値は各々13.6、15.0㎞であった。コホートAの2006と2016年における勤務市町村の人口の中央値は300.8、333.6千人であった。都道府県庁地点からの勤務市町村市役所・役場所在地までの距離の中央値は各々12.8、13.1㎞であった。よって中央値で見る限り、新しいコホートBのより都市部への分布が示された。④男性医師と女性医師別の解析では、いずれのコホートにおいても女性医師は男性医師に比して、ベースライン、フォローアップ共により人口の多く、距離の短い市町村に分布していた。⑤コホートAの1996と2006年における人口5万人以内の市町村における勤務割合は12.6%、12.0%と低下していた。コホートBの1996と2006年における人口5万人以内の市町村における勤務割合は10.4%、9.4%であり低下していた。男性医師と女性医師別の解析では、いずれのコホートにおいても女性医師は男性医師に比して、人口5万人以内の市町村における勤務割合が低かった。

① In 1996, there were 72,477 doctors (ートA) who had been registered for less than 10 years but were tracked in 2006. In 2006, there were 68,229 doctors (ートB) who had been registered for less than 10 years but were tracked for 16 years. From 1996 to 2006, the number of doctors trained decreased.
