本研究は、我々が継続している地域一般住民コホートにおける高齢者を対象に、高血圧、糖尿病、循環器疾患への罹患とフレイル・サルコペニアの進展の両者に共通する危険因子を明らかにすることを目的としている。令和2年度は平成29年度の受診者で65歳以上の342名を対象とした横断的検討を行い、男女とも下肢筋量体重比(%)とインスリン抵抗性の指標であるHOMA-IRとの間に有意な関連を認めることを確認した。令和3年度は縦断的な検討を行い、下肢筋量体重比(%)が低い群では高い群と比較してHOMA-IRが高く推移すること、HOMA-IR≧1.73のインスリン抵抗性進展の有無を従属変数とした多重ロジスティック回帰分析を行うと、年齢、性別、腹部肥満の有無、初年度のHOMA-IRで調整してもオッズ比2.17（95%CI：1.07-11.75）であり、初年度の下肢筋量体重比の低下は、将来のインスリン抵抗性進展の強いリスク因子である可能性を確認した。令和4年度は、同じく初年度にメタボリックシンドロームに該当することが、2年後の下肢筋量を予測できるかを検討した。男女別に令和元年度の下肢筋量体重比を従属変数とした重回帰分析を行うと、男性においては年齢、血清アルブミン値、ヘモグロビン値、初年度の下肢筋量体重比で調整後も、メタボリックシンドローム該当の係数βは-1.38（95%信頼区間 -2.23~-0.53）であり、有意な関連要因となった。女性においては、メタボリックシンドローム該当は有意な関連要因とはならなかった。男性においては、メタボリックシンドロームに該当することが、将来のサルコペニアに繋がる可能性が示唆された。
