高齢者への処方薬の多剤併用や長期連用は近年急増しているが、大脳容積や認知機能関連部位の萎縮との関連について検討した研究は少ない。本研究は地域在住中高年者からの性・年代層化無作為抽出者約2,200人（初回調査時40-79歳）の20年超の縦断疫学調査のデータを用い、薬効別処方薬の単剤・多剤の長期連用が大脳容積、特に認知機能関連領域の容積の変化に及ぼす影響を明らかにすることを目的としている。2020年度には処方薬服用と脳灰白質および海馬等認知症関連領域容積との関連について横断的に解析した結果、降圧薬の服用者では脳灰白質・側坐核容積が非服用者と比較すると小さく、その傾向はACE阻害薬服用者で顕著であることを明らかにした。2021年度には10年間の継続的な降圧薬の服用が中高年者の脳灰白質および海馬等認知症関連領域容積に及ぼす影響について検討し、大脳非白質の低吸収領域容積が10年間ACE阻害薬を継続して服用していた群で非服用群と比して有意に低値であることを明らかにした。2022年度には処方薬服薬数がその後10年間の海馬容積の変化に及ぼす影響を検討し、高齢であるほど、年数が経過するほど、服薬数が多いほど、海馬の萎縮は進行することを明らかにした。たとえば75歳で処方薬服薬数が0である者は両海馬容積の平均値（標準偏差）は8232.0(66.7)mm3で10年後には7723.3(67.5) mm3となり、508.7 mm3の減少を認めた。一方、服薬数が10である場合、ベースラインの両海馬容積は8065.9(136.5) mm3で、服薬数0の者と比較すると166.1 mm3少なく、10年後には6592.0(142.0) mm3となり、10年間で1473.9 mm3の減少を認めた。

In recent years, there has been a rapid increase in the number of prescription drugs and long-term use, and there has been a lack of research on the relationship between large volume and cognitive function. This study aims to clarify the relationship between capacity change and impact of long-term use of large and specific cognitive function-related domains in a 20-year-old population (40-79 years old at the time of initial review). In 2020, the correlation between the volume of cognitive disorder-related domains such as hippocampus and prescription intake was analyzed. The results showed that the volume of cognitive disorder-related domains such as hippocampus was significantly lower than that of non-users. In 2021, the decrease in ACE resistance in the 10-year period was due to the increase in the intake of ACE inhibitors, the decrease in the intake of ACE inhibitors, and the decrease in the intake of ACE inhibitors. In 2022, the number of prescriptions served was increased and the impact of hippocampus volume change in the next 10 years was discussed.
