当初の研究目的は、看護師の臨床判断力は安全な看護の提供に不可欠であり、それには患者からフィジカルイグザミネーション（以下、PE）を用いて必要な情報を得る必要がある。本研究では、先行研究で学習機会が提供されにくいと言われている小中規模病院に着目し以下を目的とする。１：中小規模病院管理者のPEに関する問題意識（阻害・促進要因）を明らかにする。２：促進要因の活用および、阻害要因に対処し学習機会を提供する実装戦略を開発する。３：実装戦略によって中小規模病院においてPEの学習機会の提供ができるか、それによって実践に改善がみられるかを検証する。しかし、令和2年、3年とCOVID-19 感染拡大による混乱があり、県外移動の制限やオンラインインタビューの実施について倫理審査委員会の許可が得られないなどがあった。そのため、初期計画を実施可能な内容に修正を行い、令和4年度は、令和3年に実施したフィジカルイグザミネーションに関する文献検討から、フィジカルイグザミネーション実装のために気づきという概念を明らかにすることを試みた。具体的には、看護実践者10名の参与観察および半構造化インタビュー調査を実施した。結果は現在分析中であるが、知識や技術だけでなく、環境も気づきの阻害、促進要因になることが示されている。気づきは哲学や認知心理学、脳科学といった領域で研究が行われている広い概念であるが、実際の実践で起こっている現象を踏まえて構造を明らかにする試みは、より看護実践的な戦略につながると考える。
