项目摘要

前年度の予備調査をもとに本調査を実施した。本調査では、多重課題場面における看護学生の視覚情報と思考の特徴を看護師との比較から明らかにした。【調査の概要】対象：看護大学4年次学生8名、看護師7名。調査手順：1)シミュレーションは複数受持ち時の予期できない多重課題（突発事象場面）とし、臨床判断の気づきおよびその時の思考に焦点を当てるため、多重課題出現初期の1分間とした。被験者には眼球運動測定装置［TalkEye Lite TTK2951］を装着し録画。2) 録画した動画を見ながら、場面における気がかり、行動の根拠、行動時の視線の意味、判断等について、半構成面接を実施し確認した。分析方法：眼球運動再生プログラムを用い、対象者ごとに着視および注視の対象領域、時間、回数を集計し、逐語録におこした面接内容を照合し、視覚情報と思考の関係を整理した。【調査結果】1）総注視時間、総注視回数、注視対象領域数の平均は看護師の方が多かった。患者別注視回数および注視対象領域数は、学生はＢ氏、看護師はＡ氏の方が多く、Ａ氏においては看護師の方が有意に高かった。2）訪室時の思考と対応：看護師、学生ともにＡ氏が急性期にあると認識し訪室していた。看護師はＡ氏の全身状態を短時間で確認し、緊急性の判断、客観的評価の必要性、悪化原因の追究、辛さへの対応がみられた。一方、学生はＡ氏の主観的な症状を主に確認し、辛さの対応への戸惑いが多くみられた。3）突発事象への対応：看護師はＢ氏の突発的な動作確認とＡ氏の観察を交互に行うものが多いが、学生はＢ氏の動作確認、介助を行うものが多かった。学生は状態の悪い患者を気にかけながらも、突発事象への対応に比重が置かれる傾向があり、気がかりの関連情報の収集スキルの獲得、優先事項の判断ができるシミュレーション教育の必要性が示唆された。2023年度は教育プログラム作成、実施、評価を行う。
