本研究では、高齢慢性疾患患者を対象に在宅と病院をつなぐ外来看護の機能を強化するために、外来看護包括的アセスメントツールの開発を目的としている。2022年度は、一般社団法人日本循環器学会「循環器病ガイドラインシリーズ」全50件のガイドライン（閲覧期間2022年3月から2022年6月）により、循環器疾患をもつ高齢慢性疾患患者への予防と早期発見を念頭に置いたアセスメント項目として、循環器疾患に特有の治療や処置に伴うリスクへの視点及び心不全、不整脈、脳血管・末梢血管を含む全身血管障害に伴う症状や兆候を抽出した。続いて、外来通院中の高齢慢性疾患患者を対象に、フレイル状態にある患者の割合と外来看護で活用可能な関連因子、フレイル状態に移行したプロセスを明らかにした。高齢慢性疾患患者へのアンケート調査により、フレイルと基本属性の関連では、女性、年齢層が高い、薬の種類数が6種類以上、受診行動で送迎がいる、支援者に体調変化を指摘をされた経験があることが、フレイル群と関連があった。またフレイル状態にある高齢慢性疾患患者へのインタビュー調査により、事例ごとの「フレイルの変化」は、予備能力が低下する下降の軌跡と、治療や参加者なりの対処・対策や意識変化に影響を受け予備能力が高まるような上昇の軌跡が示された。同時に、地域密着型の急性期病院の一般診療外来に所属する熟練した外来看護師へのインタビュー調査により、外来看護師は外来特有の環境に対し独自の工夫を見出し、支援を要する患者の抽出に努めていた。一方で、外来看護師が処置室に中央化された施設と各診察部門に配置される施設では患者抽出の病期に違いがあることが示唆され、自施設の外来機能や外来患者の特徴を踏まえ、適切な病期で支援ニーズの抽出を可能にする看護師の部署配置・看護提供体制には検討の余地があることが考えられた。
