医療的ケアを要する在宅療養児が就学する際に生じる「学校における医療的ケアの実施」に関する問題の現状と、その課題解決のための医療サービス拡充のひとつとして看護師によるケア提供のあり方を検討し、医療的ケアを必要とする子どもたちが通常学校に就学する際の体制構築のための基礎資料を得ることを目的 とした。令和4年度は、令和3年度に継続して通常学校において医療的ケアを受ける児をもつ保護者を対象として質的調査を計画した。令和2年度に実施した看護師を対象とした調査で協力を得ることができた看護職から保護者につながるよう働きかけたものの、実際には教育委員会に許諾を取ることができず断念したり、 COVID-19感染症拡大の影響により接触が困難になるなど、十分な症例を得るに至っていない。令和4年度にも試みたが、情報を得ることができていない。計画としては、医療的ケアを提供している看護職とそれを受けて生活をしている児の保護者を双方から見ることが目的だったが、現状としては困難を極めている。看護職を対象とした調査では、教育委員会の雇用もしくは訪問看護事業所との契約がほとんどであったが、教育委員会の雇用の場合はすべてが会計年度任用職員(令和2年度導入)であり、実際の勤務状況は自治体によって大きくばらつきが見られた。配置があること自体は前進であるが、配置人数や勤務時間、児や保 護者との関わり方において制約が多く、まだまだ保護者の付き添いを必要とする場面も多く見受けられた。その現状から、看護職から聞く保護者の状況は看護師配置がなかったころと大きな差はない様子である。児のケアに携わるのは主に母親であり、その母親は不規則に学校での付き添いが発生するため、就労等は難しく、結果的に母親自身の人生のあり方は後回しにされている。
