项目摘要

研究代表者は、基盤研究C（H29-R3）において女性看護職20名を対象とした面接調査を実施し、被害直後に医療機関を受診した性暴力被害女性に対し看護職が実際に提供しているケア内容と必要とされるケア内容を提示した。R4年度研究代表者は、前者の看護ケアについて論文投稿を行った。一方、後者の看護ケアとして提示された内容は、前者の看護ケアを検討する中で浮き彫りとなった課題から導かれたものであることから、必要とされる看護ケアの検討は不十分なままである。性暴力被害女性に必要とされる看護ケア内容が明らかにされなければ、看護職が適切なケア実践能力を身につけることは難しい。R4年度研究代表者は、上記の論文投稿作業と同時に、再度女性看護職を対象とした面接調査を実施すべく準備を進めた。しかし、看護職が面接調査において必要であると語るケア内容がすなわち適切なケア内容であるかの確実性を保証することは極めて難しいと判断し、海外文献において提案されている性暴力被害者への臨床ケアを基盤とし、質的内容分析を用いて日本の医療機関において被害直後の性暴力被害女性に推奨される看護ケアを明らかにすることにした。分析は、当該海外文献において支援の原則とされているsurvivor-centredの視点も考慮し、被害直後の性暴力被害女性に対する日本の医療機関における看護ケアに関する質的研究の実績をもつ研究者間で合意が得られるまで検討した。分析の結果、推奨される看護ケアとして、≪自己決定権を尊重する≫≪知る権利を尊重する≫≪情報を収集し保持する≫≪診察環境を整える≫≪健康被害を最小限にする≫≪様子や感情に留意しながら穏やかに対応する≫≪与え得る苦痛に注意を払う≫≪身体的治療とセルフケア方法を提供する≫≪心理感情的ケアを提供する≫≪安全な生活をサポートする≫が抽出された。
