项目摘要

本研究は、食物アレルギーの児を持つ親が治療法を選択する際の意思決定を支援するためのガイドを開発、それを親が利用することで、親およびその子どもの意思決定にどのような影響を与えたかを検討する。また同時にガイドの内容適切性や有用性の検討を行う。本研究は、第１段階として、食物アレルギー児を持ち経口免疫療法を選択したことのある親およびその診療に関わる医療者に対し、意思決定に関するニーズ調査を行った。さらに、欧米やわが国の食物アレルギーや経口免疫療法に関するガイドラインや先行研究の検討、経口免疫療法のＱＯＬに関するシステマティックレビュー等をもとに、意思決定ガイド「いっしょに考えよう食物アレルギーの治療法‐治療法を検討されている皆様へ‐」を開発した。第２段階として、食物アレルギーの治療法選択の経験のある親を対象に、本意思決定ガイドの内容適切性評価を行った。本研究の結果、意思決定ガイドの親の受け入れは概ね良好であったが、一部難しい表現のある個所などについて、食物アレルギーの診療を行っている小児科医、意思決定の研究者らで構成される、意思決定ガイドの開発グループ内で議論を重ね、改良を行った。現在、以上の経過を得て開発した意思決定ガイドの介入研究として、実際に食物アレルギー児を持ち治療法を選択する段階にある親および食物アレルギー児を対象に、介入前後の質問紙および介入後のインタビュー調査を実施中であり、混合研究法にて分析を進めている。
