本研究の目的は、介護医療院における医療・介護の連携を基盤とした充実した看取りを具現化するために、医療と介護の協創によるエンドオブライフケアモデルを構築することである。そのために、まず、介護医療院における医療・介護の連携による看取りケアの具体的実践内容とその実践により得られる成果を抽出し、看護・介護の連携によるエンドオブライフケアの構造を明らかにすることとし、島根県東部の介護医療院3施設のケアスタッフ（2施設からは看護職員および介護職員それぞれ1名、1施設から介護職員1名の合計5名）を対象としたフォーカスグループインタビューによるデータ収集を行った。得られたデータを分析した結果、【介護医療院における看護・介護の連携による看取りケアの具体的実践内容】として＜看護・介護での受け持ちをを最期まで続ける＞＜日々のカンファレンスとは違う看取りのカンファレンスを開催・運営する＞＜利用者の日々の様子を看護・介護で一緒に見る＞＜看護・介護が協力してできるだけ入浴できるようにする＞＜日常の様子や病状の変化に細やかに気づき合う＞などの10項目、【介護医療院の看取りの質向上に資すると考えられる看護・介護の連携による看取りケアの具体的要素】として＜家族がケアに参加するなどして家族と本人をつなぐ＞＜普通の日常生活にできるだけ触れられるような機会を増やす＞の2項目、【看取りにおいて、看護・介護が連携したケアを提供することにより得られる成果】として＜看護と介護の協働によって最期までその人らしく生きる支援ができる＞＜連携して工夫したケアをすることによって利用者の苦痛緩和につながる＞などの4項目が抽出された。
