项目摘要

本研究の目的は、外国人患者のケアにおいて、言葉の障壁が低減した場合に、看護職の文化能力（cultural competency）がどの程度高まるか、言葉の問題とは独立して残される文化能力の領域、すなわち教育や研修が必要な文化能力の領域は何かを明らかにすることである。令和2～3年度は、病院の医療通訳者および自動翻訳機導入と看護職による活用の実態を把握するとともに、看護職の文化能力の関連要因を明らかにするため、外国人居住者が多い7都道府県における病院を対象に質問紙調査を実施した。33病院、403人から返答が得られ、病院の医療通訳者雇用や自動翻訳機導入、看護師の文化能力に関する実態が明らかとなった。また、重回帰分析により、海外在住経験、異文化感受性、異文化コミュニケーションなどが文化能力に肯定的な影響を及ぼしていることが明らかになった。令和4年度は、病院において自動翻訳機を導入して外国人患者に活用してもらい、文化能力が変化する分野は何かを明らかにすることを目的に介入研究を行った。同意が得られた5病院の、外国人患者がいる病棟、外来、および救急部門を対象とした。介入期間を2か月とし、ベースライン開始時、介入開始時、介入終了時、フォローアップ終了時に質問紙調査を実施した。文化能力の測定には、日本語版異文化間感受性尺度、日本語版文化コミュニケーション尺度、日本語版看護文化能力尺度を用いた。分析の結果、自動翻訳機導入後、文化能力尺度における「文化的な技術」得点が上昇している者が多く、個人間に差がみとめられた。しかし、外国人患者5人以上のケア経験をもつ看護師は少なく、変化要因の把握が困難であったため、今後外国人患者対応経験および自動翻訳機使用経験が多い看護師を対象に、インタビュー調査を実施することとした。併せて、訪問看護ステーションを対象とした調査を実施し、分析を進めていく予定である。
