项目摘要

2021年度に明らかにした健康施策形成から評価までのプロセスを参考に、2022年度はインタビュ調査を行った。事業場規模の違いを考慮した合意形成のプロセスを明らかにすることとした。インタビュ調査の目的は、企業組織内で健康ニーズを施策化する合意形成のプロセスに関与した経験がある保健師を対象に、円滑に合意形成を行うために把握した情報や、企業が健康ニーズを施策化する際に合意形成を行うための産業保健師の支援および困難・課題を明らかにすることとした。研究対象者は、健康ニーズを施策化するプロセスに関わった経験がある実務経験5年以上の常勤雇用の産業保健師とした。産業保健師が所属する事業場規模は、産業医が常勤雇用されている企業、産業医が非常勤で雇用されている中小企業として、産業保健体制に偏りが出ないようにした。また、産業保健師の実務経験は5年目以上とした。インタビュは、2022年11月下旬から12月下旬に実施した。11名の産業保健師にインタビュ調査を実施し、経験年数は平均21年であり、健康課題を解決するために自立して対応し、組織へ働きかけるレベルである経験豊富な保健師であった。所属する組織の産業保健体制として、産業医常勤雇用が6名、非常勤雇用が5名であった。現在結果の分析中であるが、ニーズを施策化するプロセスでは、産業保健師はPDCAサイクルを回すことで、(P)計画立案、(D)実施、(C)評価、(A)改善のプロセスを段階的に踏んでいた。並びに円滑な合意形成ができるよう、会社のニーズ、企業の経営決定者の健康への認識の確認や働きかけをすることで、他部門・多職種を巻き込みながら組織マネジメントを実施していた。更に、日常的な実践活動や施策化の一連のプロセスを通して、健康の保持増進が実現できる企業風土が醸成できるような働きかけをしていることも推測され、詳細について今後分析を進めていく。
