糖尿病高齢者を支援する訪問看護師、介護支援専門員、行政保健師からインタビューを行った結果について質的統合（KJ）法を用いて分析を終了した。このうち、訪問看護師のインタビュー結果について、演題名を「中山間地で暮らす高齢糖尿病患者の在宅療養継続に向けた訪問看護師の思いと支援上の課題（帆苅・上原・原・中村）」とし、第27回日本糖尿病教育・看護学会（2022年9月17日～18日：ハイブリッド開催）で口述発表した。訪問看護師は「食を大切にした関わりと体調変化・家族への配慮」と「出来うる限りの疾病管理と緊急時対応」という2側面から訪問看護の実践を行っていた。しかしその中で、「自己管理の不十分さと家族の意向確認の難しさ」という「訪問看護の窮状」があることと、「思いを阻む人手不足と制度上の障壁および実施困難な医師の指示」という「訪問看護師のジレンマ」を抱えていた。これらの訪問看護を支えるために、2つの面からの取り組みの必要性を感じていた。一面では、「訪問看護師のジレンマ」故に「身体要因と療養環境に応じた地域の繋がりによるサービスの補完」という「在宅療養のための後方支援の必要性」を感じており、他方では、「訪問看護師のジレンマ」とともに「訪問看護師の窮状」故に「自己管理および家族と多職種の連携・協働による補完」という「在宅療養継続のための基本体制の構築の必要性」を感じていた。中山間地で暮らす高齢糖尿病患者が在宅療養を継続するためには、家族や医師と共通認識を図り、地域の繋がりを活かしたサービスや家族を含めた他職種連携・協働を図っていく必要性が示唆された。
