项目摘要

本研究課題は、A)医療事故に関する事実の収集精度を高めるため、事実をインシデントレポート（IR）に正確かつ十分に記述するための指標を明確化する。また、B)IRで記載しきれない事実情報（不足情報）をヒアリング等により同定する手法を確立する。本年度は、以下の2点を実施した。A）IR記載指標の有効性検証昨年度は指標として、5W1Hを含む、目撃した事・物を含む、事象の時系列で箇条書きにする、各文に主語を含む等を明らかにした。さらに、それを用いて研修医、新人看護師、指導医にIR記述研修を実施した。本年度は、同研修によって当院内のIRの事実情報記載に変化が生じたこと、さらにその定着率について検証した。結果、研修直後のIRは、報告者の行動、作業手順、報告者が置かれていた環境について明記されるようになった。一方、研修半年後のIRは直後に比べて、環境、本来の手順、チームの行動についての記述率が低下し、指標の持続効果に課題が生じた。そこで、研修医を対象にフォローアップ研修を実施し、指標の再周知を行った上で、医療安全の代表モデルであるP-mSHELLに基づき、患者背景、本来の手順、薬品や機器名、環境、自身の行動、チームの行動を記述するよう推奨した。次年度は、フォローアップ研修後の指標の持続効果（長期的効果）について引き続き、検証を行う。B）IRの不足情報の同定ノウハウ明確化事故分析経験者が不足情報について、どのように事故当事者にヒアリングをしているか、そのノウハウを明らかにする。本年度は、事故分析経験者11名と未経験者15名を対象に調査を開始した。具体的には、対象者に架空のIRを閲覧させ、ヒアリングで質問したい事項を列挙させた。本調査結果は分析途中である。次年度は調査結果を基に、不足情報の同定手法をまとめ、当院内の安全対策部の職員と各病棟の安全管理担当者に周知し、手法の妥当性を検証する。
