本研究は、老年期うつ病に対する反復経頭蓋磁気刺激（rTMS）治療の有効性、老年期うつ病に併存する認知機能障害でのアセチルコリン機能・GABA機能について健常群との比較、およびアセチルコリン機能・GABA機能についてrTMS治療による変化と関連性について調べることを目的としている。具体的には老年期うつ病におけるrTMS治療前に、精神症状・認知機能評価とともに短潜時求心性抑制（SAI）と二連発経頭蓋磁気刺激（ppTMS）を測定してアセチルコリン、GABA機能を評価し、同年代の健常対照者との差異を明らかにする。また、rTMS治療後も同様の評価・測定を行い、治療反応性とアセチルコリン、GABA神経伝達機能の関連を検討し、老年期うつ病におけるrTMS治療の生物学的基盤の解明と反応予測指標の確立を目指すことが本研究の目的である。2020年度に当院でのrTMS治療機器NeuroStar(Neuronetics社)の導入後、治療担当医師の技法を獲得し、rTMS治療についてrTMS適正使用指針（日本精神神経学会）に準拠し、保険治療を開始した。2021年度、現在まで進捗として、県内唯一のrTMS治療の場として、疾患群の治療および研究のリクルートを行い、5例の測定・治療を終了している。2022年度、コロナ下において、入院治療のrTMSが滞ったため、新たな症例の組み入れが出来なかった。国内学会・研究会への参加にて他施設の状況の収集を図った。

The purpose of this study was to compare the efficacy of repeated cranial magnetic stimulation (rTMS) therapy in elderly patients with cognitive impairment in elderly patients. Specific measures include: pre-rTMS treatment, assessment of mental symptoms, cognitive function, short-latency cardiac inhibition (SAI), and dual cranial magnetic stimulation (ppTMS); assessment of GABA function; and differences between healthy subjects of the same age. The purpose of this study is to investigate the biological basis of rTMS therapy and to establish predictive markers of therapeutic response. In 2020, after the introduction of the rTMS treatment machine NeuroStar(Neuronetics) in the hospital, the techniques of the treating physician were acquired, and the rTMS treatment was properly used (Japanese Psychiatric Association). In 2021, the only rTMS treatment field in the country has been developed, the treatment of disease groups and research has been conducted, and 5 cases have been determined and treatment has been completed. In 2022, the number of patients admitted to the hospital was increased, and the number of new cases was increased. The National Society for Research and Development participated in the collection of other facilities.
