「皮膚筋炎の筋膜炎における神経系の役割を明らかにすること」を目的として以下の研究を行った．・筋骨格系MRIによる筋膜病変/筋肉内病変の解析骨格筋MRIにより抗MDA5抗体陽性皮膚筋炎28例の筋膜病変および筋肉内病変の有無を解析し，肺病変および間質性肺疾患の予後との関係を調べた．画像的に筋肉内病変を有するグループと，筋肉内病変を有さないグループを比較したところ，前者のグループでは間質性肺炎の程度が有意に軽く，致死率も低かった．また，筋肉内病変を示した部位の割合が高い症例ほど，間質性肺炎の肺病変の広がりが少なく，両者は有意な負の相関を示した．28例中，4例が致死的な経過を辿った．この4例はいずれも筋膜病変を有したが，筋肉内病変は見られなかった．これらの結果から，抗MDA5抗体陽性皮膚筋炎では，何らかの要因により筋膜の炎症が筋肉内へ波及しない病態が，肺病変の増悪に関与していることが示唆された．骨格筋MRIにおける筋膜病変を病理学的に解析するため，en bloc biopsy法により採取した筋膜/筋肉を病理学的に解析したところ，筋膜およびそれに隣接する筋肉において，血管周囲に炎症細胞浸潤が見られた．・免疫組織染色による患者の筋膜における神経ペプチドおよびその受容体の解析en bloc biopsy法により皮下組織，筋膜，筋肉を一塊に採取した検体を病理学的に解析し，神経ペプチドであるcalcitonin gene-related peptide(CGRP)およびsubstance P(SP)の局在を明らかにする目的で，抗CGRP抗体，抗SP抗体による免疫組織化学染色を施行した．

The purpose of this study is to investigate the effects of dermatotendinitis and fasciitis on the nervous system. The relationship between fascial and muscular diseases in 28 cases of cutaneous tendinitis and interstitial lung disease was studied. The severity of interstitial pneumonia was significantly lower than that of the control group, and the mortality rate was significantly lower. In 28 cases, 4 cases of interstitial pneumonia were fatal. 4 cases of internal diseases The results showed that anti-MDA-5 antibody positive skin tendonitis was caused by inflammation of fascia and muscle tissue, and lung disease was increased. MRI of bone and tendon was used to analyze the pathology of fascia and tendon. Inflammatory cell infiltration was observed around the blood vessels. Immunohistochemical staining was performed for the detection of calcitonin gene-related peptide(CGRP) and substance P(SP) in the fascial tissue of the patient. Immunohistochemical staining was performed for anti-CGRP antibodies and anti-SP antibodies.
