本年度も、前年度に引き続いて、心臓血管外科手術における術後せん妄および術後認知機能障害の発生と、術前・術後に採血した検体から検出されるリン酸化ニューロフィラメント重鎖の濃度との関連を解析した。新型コロナウィルス感染症流行に伴い、これを理由とした認知機能検査の応諾率が悪く、有効な定量的認知機能検査を行うことができなかったが、研究者所属施設の定時手術の半数程度にあたる約30例において、血液検体の採取を行うことができた。現在、当該検体は外注検査中のため、リン酸化ニューロフィラメント重鎖の濃度についての定量的解析を待つ状態にある。研究計画段階では、文献的には半数程度の症例で術後せん妄が出現することが期待されたが、CAM-ICUによる術後せん妄診断に至ったのは数例に過ぎず、その他数例において幻視・幻聴がみられるにとどまった。人工心肺装置の使用や低体温循環停止などが中枢神経系へのダメージのリスク因子として想定されたにもかかわらず、現時点ではこれらを肯定する検査結果は得られていない。これらの点から、中枢神経系へのダメージの観点では、心臓血管外科手術の安全性を説明する結果が得られる可能性が高く、本年度に解析を完了させる計画にある。

In the study program, half of the cases in the literature were diagnosed postoperatively, and some cases were diagnosed postoperatively in CAM-ICU. The use of artificial cardiopulmonary devices, hypothermia, circulatory arrest, and central nervous system factors were determined at the current time point. This year's analysis of cardiovascular surgery is completed.
