口腔舌扁平表皮癌は早期がんであっても高率に頚部リンパ節転移を起こす。本研究は口腔舌扁平上皮癌における頸部リンパ節転移予測因子について調べることを目的とした。大腸癌のリンパ節転移予測因子として知られる簇出（Budding）を中心に病理学的予後予測因子の検討を行い、がん微小環境に注目し分子生物学的検討を行うことで舌癌の頸部リンパ節転移のメカニズムを解明し、予後予測による新たな治療戦略を確立することを最終目標とした。昨年度は、当施設における舌癌症例70例のBuddingを評価した。Buddingの検討する際、検者による評価のばらつきを認めた。そこで、再現性及び検者間信頼性が高いことで知られるTumor stromal ratio（TSR）に注目し、同症例に対して評価をおこなった。ここで得られたBuddingおよびTSRと臨床病理学的因子とで多変量解析を行ったところ、TSRが独立した頚部リンパ節転移予測因子であることが示された。この結果は国内学会で発表を行い、英文誌に掲載された。これに加えて昨年度はがん微小環境において関与が指摘されている交感神経の腫瘍先進部での増加の有無をTyrosine Hydroxylase (TH)染色にて評価し、臨床病理学的因子との関連を解析した。また凍結切片よりRNAを採取した新しい14症例で神経浸潤の有無での該当RNAの発現を比較した結果について解析を進めている。この結果を現在学会発表および英文誌へ投稿準備中である。

Flat epidermoid carcinoma of the mouth and tongue is characterized by early stage, high rate and high incidence. Last year, there were 70 cases of tongue cancer with Budding syndrome. Budding is very important to the international and international community. The factors of bed pathology were analyzed by multivariate analysis of the factors of Budding pathology and TSR pathology. The independent factors of TSR were transferred to factors. The results show that there is a list of students in the domestic society, including English and English. In the last year, there was an analysis of the factors of Tyrosine Hydroxylase (TH) staining and bed pathology in the microenvironment. In 14 cases of new symptoms, the results of RNA analysis were improved when RNA was detected. The results show that the students are now preparing to submit their contributions in English.
