项目摘要

顎顔面領域の形態的・機能的改善を目的とした顎変形症に対する外科的矯正治療では、術後の長期安定性に十分配慮して治療計画を立案することが必須である。また、矯正歯科治療の診断や治療方針立案時には、術後の長期経過およびQOL改善に大きく寄与する舌や口唇・頬部軟組織などの機能的要因についての詳細な検討が必要である。骨格性開咬症は顎骨や歯列の著しい垂直的異常とともに、顎口腔機能の低下を示すことが多い。外科的矯正治療の開咬の再発率は約20%との報告もあり、その病態は顎変形症の中でもとりわけ難治性と考えられている。骨格性開咬症の嚥下機能は、顎骨の不調和に起因し健常者とは異なる軟組織運動様相を示すと考えられる。しかしながら、骨格性開咬症の嚥下機能を舌や口唇・頬部軟組織の運動という観点から同時に検討した定量的評価は少なく不明な点が多い。本研究では、骨格性開咬症の嚥下時における舌および口腔周囲軟組織の機能的特徴と連動様相を解明することを目的に、舌圧と口唇・頬圧の同時測定法の確立、次に、骨格性開咬症における舌圧と口唇・頬圧の同時測定データの解析と顎顔面形態と嚥下機能の連動性との関連を検討することとした。2022年度は、舌圧ならびに口唇・頬圧の発現と密接に関連すると推測される口蓋形態に着目し、嚥下時の舌圧および顎顔面筋群筋活動と口蓋形態との関係性について検索した。その結果、嚥下時の舌圧および顎顔面筋群筋活動は口蓋深さや口蓋幅径と関連することが示唆された。また、個性正常咬合者および顎変形症患者における嚥下時の舌圧と顎顔面筋群筋活動を同時測定データを解析し、嚥下時の特徴について検索した。これらの研究成果については、国際誌に投稿中、または投稿予定である。現在、舌圧と口唇・頬圧の同時測定への準備を進め、口唇・頬圧の測定部位や咬合を妨げない装置の設計について検討している。

In 2022, the relationship between tongue pressure, lip pressure, cheek pressure and adhesion was estimated. The relationship between tongue pressure, jaw muscle activity and flap morphology was estimated. The results show that the tongue pressure and jaw muscle activity during swallowing are related to the depth and diameter of the mouth. The tongue pressure and jaw muscle activity of patients with normal occlusion and jaw deformity were measured simultaneously, and the characteristics of swallowing were detected. The research results of this event are in the process of being submitted to the International Journal, and the submission is scheduled.
