项目摘要

唇顎口蓋裂は、口唇外鼻形態に特徴的な変形を伴う疾患であり、患者のQOLと密接に関係する問題である。唇裂治療の目的は対称的な口唇外鼻形態の回復と生理的機能の回復である。しかし、変形の度合いは個人で大きく異なり、術後の口唇外鼻形態や将来の形態変化について一概に予想することは困難である。本研究では、唇裂患者の口唇外鼻形態について、新規アルゴリズムを応用し出生時から唇裂手術前後など経時的な変化を三次元的に計測・評価することで治療効果を分析、フィードバックして効果的な治療方針の確立を目的とする。また、経時的顔貌の予測モデルを可視化することで、患者・家族の治療への不安低減および患者のQOL向上を目指す。初診時からNAM装着前、装着後の口唇形成術直前、口唇形成術後の唇裂患者の口唇外鼻形態の三次元画像取得および口唇外鼻形態の計測を進めている。2022年度は対象となる唇裂患者の顔貌を、非接触型三次元撮影装置にて初診時から各時期に撮影し、撮影後、顔貌形態、特に口唇外鼻形態の三次元分析を行うために、取得した顔面画像から三次元画像を構築している。今年度はハイスペックPCを購入し、随時撮影データから三次元顔面画像構築を行い、専用ソフトを用いて三次元分析をすすめた。三次元分析は、三次元画像において解剖学的計測点を用いて、鼻尖点の位置、鼻翼の挙上程度、鼻翼角や鼻柱角計測を行う。また、今年度は、三次元画像構築において必要なソフトウエアのアップデートに伴い、使用方法のについてや、計測方法・分析方法についての勉強会への参加をおこなった。また、共同研究者との研究打ち合わせを通して、効果的で正確な分析方法などのブラッシュアップなどをおこなった。
