项目摘要

日本における医師の地域偏在は深刻である。そこで、1972年に自治医科大学は医師不足地域の医師を確保するために開設された。自治医科大学卒業生は医学部卒後9年間を出身都道府県のへき地診療所を含めた医療機関で勤務し、出身都道府県の地域医療を担うことを義務付けられている。自治医科大学は「医療の谷間に火を灯す」というアドミッションポリシーを掲げ、生涯にわたる地域医療への貢献を教育している。卒業生が地域医療を支える役割を果たす一方、「地域医療＝自治医科大学」の図式ができたことで、他大学出身の医師の地域医療への意識が高まりにくく、さらに地域の医師不足が深刻化した。さらなる医師の地域偏在の解消策として大学医学部に地域枠入学選抜制度が設けられた。しかし、医学教育の視点が欠如し、地域枠医師の意思に関らず数合わせのために派遣される現状は、地域枠入学選抜制度の弊害といっても過言ではない。実際、研究者が勤務する和歌山県立医科大学では卒後9年間の県内勤務を義務付けられた地域枠入学者が約3割を占め、一部の地域枠学生・医師は、自らの意思に反して派遣されたと感じる弊害が生じ、そのことが医師キャリア支援を困難としている。そのため、地域枠入学者の量的・質的調査結果の一般化することが、当事者目線からみた医学教育の改善及びキャリア形成支援の開発に資する。当事者の目線から現状の入学選抜の弊害を明らかとするとともに、当事者が必要としている支援の要素を抽出することで、当事者目線からみた医学教育の改善及びキャリア形成支援の開発を行うことを目的とした。

Japan's doctors are biased in depth. In 1972, the Autonomous Medical University was established to ensure the safety of doctors in areas where doctors were insufficient. A graduate of Jichi Medical University has been working in a local clinic in the prefecture for nine years after his death from the Faculty of Medicine. He has been working in a medical institution and has been working in a local medical clinic in the prefecture. The Autonomous Medical University has contributed to the development of regional medical education through the development of medical science and technology. Graduated students have a high awareness of regional medical care, and the shortage of doctors in the region has deepened. The regional selection system for university medical departments has been established. The viewpoint of medical education is insufficient, the meaning of regional doctors is related to the number of combinations, the status quo and the disadvantages of regional admission system are not correct. During the 9 years since the death of Wakayama Prefectural Medical University, researchers have been assigned to work in the field, and about 3 students have been enrolled in the field. Some students and doctors have been assigned to work in the field. There are problems and difficulties in supporting doctors. Generalization of quantitative and qualitative survey results on enrollment in different regions, and development of support for the improvement of medical education and the development of health care. The goal of the party is to improve the quality of medical education.
