表情の変化は、表情を表出するものと、視覚的に認識する者との間で成立する非言語的コミュニケーション手段となる。ナイチンゲールは、看護師の基本は、患者が何を感じているかを患者に大変な思いをして言わせることなく、患者の表情に表れるあらゆる変化から読み取ることであると述べている。看護ケアは、看護の知識や経験のみならず、看護師個人の直感や思考が反映されたものであり、患者に心地よい、安楽、爽快感などの精神活動の変化が生じる。本研究の目的は、看護ケアに伴う、心地良い、安楽などの精神活動のもたらす表情形成プロセスについて混合研究を用いて分析し、患者理解を深めると共に、よりよいケアの創出につながるようエビデンスに基づいた新たな表情観察法の提案に向け、検討することである。看護ケアに伴う心地良い、安楽などの表情の変化について、生理学的手法により測定し、客観化した状態で特徴の抽出と、その変化過程の可視化を試みる。次に、質的研究として、看護師の患者の表情を読み解く現象について看護の実践知から深層構造を明確にする。これらの研究を統合し、看護ケアと表情形成プロセスのつながりを見出し、エビデンスに基づいた新たな表情観察法を提案するものである。研究期間の後半においては、表情観察に関するグループフォーカスインタビューや半構造化面接を検討していた。本年度には、看護学生10名を対象に、表情に着目した場面について半構成面接を実施した。これを基に、表情の要素を顔の部位ごとに分類し、また、推察した感情の種類をまとめ考察した。また、におい適用時における表情筋の電位を測定し、その変化と主観的反応、実験方法などについて検討するためのパイロットスタディを実施した。
