项目摘要

外来通院している進行がん患者は約 60%が疼痛を自覚し、そのうちの約 20%は中程度から高度の疼痛を経験している。近年がん患者が療養する場は、超高齢化社会の到来及び社会保障費の増大による社会システム変化により在宅療養が推進されている。そのため、入院中や外来で教育を受けた患者は、その後のセルフマネジメントを自分自身で実施せざる得ない状況であり、耐え切れないほどの苦痛を伴ってから医療者に連絡する患者が存在する。安心した在宅療養を継続する新たな地域ケアシステム構築が喫緊の課題である。課題には、患者が入力した日々の症状を可視化することで、患者自身が悪化を早期発見し、異常時には医療者から支援される遠隔看護システムの活用が見込める。本研究の目的は、在宅進行及び終末期がん患者を対象に、在宅課題に沿った信頼性及び妥当性のある「がん疼痛緩和を目的とした遠隔看護システム」を開発し、無作為化比較試験により、がん疼痛の程度・セルフマネジメント能力・QOL・疼痛とオピオイドに対する懸念・経済評価を評価指標とし、有効性を検討することである。研究は、【Step1: 在宅版がん疼痛緩和遠隔看護システムの開発】を行い、【Step2:インターフェイス評価】によりシステムの利便性を評価し、【Step3:無作為化比較試験及びインタビュー調査によるミックスドメソッド】を実施する。 2020~2021年度は、Step1を実施し、Step2について所属機関の倫理審査を受けた。2022年度はStep2を実施し、医療者及び患者による評価を受けた。その評価を受けたものを改修し、Step3について所属機関の倫理審査にて承認を受けた。2023年度はStep3を実施していく。
