本研究の目的は、子育て支援の出発点として重要である母子健康手帳交付の場面で、熟達した技術をもつ保健師が展開する妊婦個別指導の看護実践から卓越した看護アセスメントを解明することである。昨年度に続き、富山県内で実践する熟達保健師への調査に向けて、研究概要の説明・参加者募集を進めた。本研究の対象者が所属する部署は感染症対策業務の関連部署であり、これまで保健事業や役割分担等の体制の見直しが行われ、母子健康手帳交付時に行う妊婦個別指導においても担当者など実施方法を変更する市町村もあった。一方で、地区担当保健師による対面実施を重要視する等の理由から、従来の実施方法を継続している市町村もあった。さらに、妊娠後期の妊婦個別指導など、地域の実情に応じて妊娠期における行政保健事業の充実が図られていた。これらの状況を鑑み、母子健康手帳交付時の妊婦個別指導における支援の質の向上は、その後に続く子育て支援にも大きく影響を及ぼすことから、これまで以上に強く求められる傾向が伺われた。したがって、母子健康手帳交付時の妊婦個別指導担当者においては、本研究への関心や参加の意向をを示す状況が確認された。母子保健における課題の改善、妊娠・出産・子育て期の多様なニーズに対応するために、保健事業の充実と並行し、保健事業を担う母子保健・子育て支援領域に関与する専門職の実践力向上が不可欠である。よって、卓越した看護アセスメントを伝達・共有・教育できる形式知として解明することが求められていると捉えることができる。
