本年度の研究課題として、「認知症高齢者の食事摂取にかかる一連の過程を客観的に測定する評価ツール」の作成を目指した。画像に映っている人物の姿勢をディープラーニングで推定するシステムを使用して、姿勢計測を行うとともに、誤嚥しやすい姿勢角度を推定し、「姿勢の崩れ」評価を行う。具体的な計画として、若年者などの健常者を対象に、さまざまな活動の開始前から終了時までの姿勢を計測し、誤嚥しやすい「姿勢の崩れ」とみなす姿勢角度を推定し、複数の専門職に、「姿勢の崩れ」評価を依頼し、その妥当性を検証するように考えた。食事時における誤嚥しやすい姿勢の検知に関しては、客員研究員として在籍していた大学院博士課程の元指導教員の研究室に協力を求めた。正常な座位姿勢から、頸部後屈位、および仙骨座りへとそれぞれ段階的に変化させ、その時の姿勢を横から撮影しし、骨格画像を推定するシステム(open pose)を使用して、被験者の体の各部の座標をとった。2名の専門職に、通常画像と骨格画像を提示し、誤嚥の危険性が高い場合は停止、誤嚥の危険性が低い場合は、継続と判定してもらった。2者間の判定結果は、再現性が高く、姿勢検知技術を活用した誤嚥リスク評価ツールは、介護者が食事を継続または中止を適切に判定するために活用できる可能性が示された。しかし、本研究課題が、「食事」を前提とした取り組みであることから、食事場面の被験者実験を行うことでCOVID-19の暴露の危険性が高まるため、被験者への安全性を考慮した上で、食べるという食品を用いた動作を避けたこと、また、食事中にとりやすい姿勢を他の動作で代用した実験となった。したがって、今後は、実際に、要介護高齢者の生活の場で、食事時の姿勢の崩れが測定できるよう、研究をすすめたいと考える。
