项目摘要

児童虐待の相談件数は年々増加の一途を辿り、妊娠期からの児童虐待防止対策が推進され、社会的ハイリスク妊婦への対応は一層重要で急務な課題である。本研究では中枢中核都市の久留米市(人口30万人)において教育・行政機関が連携して、社会的ハイリスク妊婦と出生児の現状と課題を抽出することで児童虐待予防や養育支援につなげていく。本研究は１社会的ハイスク妊婦とその出生児に関する実態調査２ハイリスク因子の重み付け評価３行政機関とアカデミアの連携により母子保健活動の向上をはかることを目的としている。2021年度は目的①を達成するために、久留米市に提供を受けた母子保健情報の解析を行なった。2017年10月以降に出生し、久留米市で妊娠届出を提出している妊産婦のうち、2020年2月から12月末までに出生児が久留米市での1歳6か月健診を受診した延べ2,130名を抽出した。そのうち社会的ハイリスク妊産婦としてランク評価を受けたのは334名であった。母体の平均年齢は30.8才、出生児の平均在胎週数は38週6日、平均出生体重2996gであった。訴え多い・不安が強い、精神科的疾患、未婚、育児・相談相手なしの順でリスク要因の上位を占めた。過去の我々の医療機関での調査では経済的困窮が最多であったが、経済的困窮は多い順で6番目であった。行政が所有している母子保健情報を集計・分析することでその地域が抱える社会的ハイリスク妊産婦のリスク要因がみえてきた。これらの結果は2021年11月に久留米市と久留米大学間で連携会議を行なった際に中間発表として行政に報告を行なった。R4年度は5月より産休・育休取得のため、研究を一時中断した。次年度は更なる詳細な解析・検討を行なっていく予定である。
