项目摘要

本研究の要旨は以下の4点である。①左室壁運動が全体的に高度低下している症例において真の左室収縮能(Ees)を非侵襲的に求める。このようにして「高度かつ同程度の左室収縮不全の60症例をエントリーする。②経胸壁3次元心エコー法により僧帽弁尖組織の面積・弁輪周囲長および弁尖の縦方向の組織長を独自のソフトを用いて計測する。③症状および心エコー図検査により弁尖テザリング・健常者30名および全体的かつ高度に左室収縮能低下の60例を対象とする。左室収縮能低下例を心不全がより重症の30例およびより軽症30例に群分けし、心不全重症度を決定する。④上記より「僧帽弁尖組織延長が弁尖テザリング・機能性僧帽弁逆流（FMR）および心不全を軽減するか？」検討する。本研究の対象は健常者30名および全体的かつ高度に左室収縮能低下の60例を収集する。①－④は達成し、現在継続してそれぞれの標準的な左室機能・僧帽弁機能・心機能の評価、左室Eesの算出、3次元心エコー法による僧帽弁尖組織サイズ（組織延長）の定量および統計解析を進めている。本研究で得られた各心エコー測定値等を先行研究で既に報告されている結果と矛盾がないか等を確認し、更なる学術的意義の向上を目的とした心不全イベントのリスク等の予後評価の追加を検討している。また本研究実施過程の症例収集に際し、COVID-19流行の影響で症例収集がやや遅れた状況から労働者の医療機関の受診頻度が減っている可能性に着目し、COVID-19流行下における地域的な感染拡大と労働者の治療中断の関連の検討(横断的研究)を行い、論文名【Spread of infection and treatment interruption among Japanese workers during the COVID-19 pandemic: A cross-sectional study】をFrontiers in Public Healthで報告した。
