项目摘要

持続可能な開発目標（SDGs）において推進されているユニバーサルヘルスカバレッジ（UHC）は、『全ての人々が適切な予防・治療・リハビリ等の保健サービスを支払い可能な費用で受けられる状態』を意味する。UHC進捗のモニタリングのための指標として、Out-of-pocket支出（OOP: 医療費自己負担分）が世帯総支出の25%を超える破滅的医療支出に陥る世帯の比率がある。本研究では、ベトナムにおいてOOP支出の国際定義に含まれない交通費等の『隠れたOOP支出』を含めた場合の破滅的医療支出世帯の比率を推定する。併せて、現行の同比率の過小評価の程度を考察する。具体的には、既に獲得した科学研究費助成事業『研究活動スタート支援』により実施する１保健施設（省病院）の入院患者の世帯を対象にした『隠れたOOP支出』による破滅的医療支出世帯の比率を推定する調査を、一般地域住民を対象とした世帯訪問に基づく調査に拡張・発展した形で行う。これにより、より一般化可能性が高い形でしかも制度がより高い『隠れたOOP支出』の実態が明らかになる。また、本研究の結果を基に、近い将来『隠れたOOP支出』に関するMulti-country studyの実施の礎とする。さらに、新型コロナウィルスによる住民の受診行動変容や世帯レベルの医療支出へのインパクトの評価もサブ分析として試みる。2021年度は当初データ収集を行うことを想定していたら、調査対象国であるベトナムが新型コロナウィルス流行のため、調査の準備活動を行うことに切り替えた。準備活動の詳細は【現在までの進捗状況】を参照されたい。

UHC means "all people are entitled to appropriate prevention, treatment, treatment, and other health care services." UHC progress and Out-of-pocket expenditures (OOP) are 25% of total expenditures and the proportion of Out-of-pocket expenditures exceeding the total expenditures. This study estimates the international definition of OOP expenditure, including transportation costs, and the ratio of health care expenditure to total expenditure. And the current ratio is too small to evaluate the degree of investigation Specific research expenditures include research activities support, health care facilities (provincial hospitals), estimated health care expenditure ratio, general population, research activities support, and basic research. The general possibility of the system is high, and the actual state of the OOP expenditure is high. The results of this study provide a basis for the implementation of Multi-country studies related to future OOP expenditures. In the meantime, the analysis of the evaluation of the medical expenditure of the residents in the new type of medical treatment is carried out.
