本研究では、National Clinical Database (NCD)を用いた実態調査を行い、男性乳癌の臨床病理学的特徴を評価することとしている。男性乳がんの臨床病理学的 特徴に関してNational Clinical Databaseを用いた大規模研究の研究計画書を作成し倫理委員会の承認を得た。NCD登録が開始された2012年から2018年までにNCD に登録された全乳癌患者を対象として、年齢、病期、術式、エストロゲン受容体、プロゲステロン受容体、HER2の発現、家族歴、合併症、全身治療歴について収 集し、男性乳癌と女性乳癌を比較した。データ抽出、解析が完了し、2021年度末にBreast Cancer誌へ投稿　、掲載された(Breast Cancer. 2022 Nov;29(6):985-992.)。本研究では、2012年1月から2018年12月までの間に、日本のNational Clinical Database（NCD）で乳がんと診断された患者を対象とした。男性(MBC)3780例（0.6％）、女性(FBC)590,536例（99.4％）の計594,316例が評価された。MBCとFBCの診断時の年齢中央値はそれぞれ71歳（45-86、5-95％）、60歳（39-83）であった（p < 0.001）。MBC症例はFBC症例より臨床病期が高かった。乳房温存手術はFBCでより頻繁に行われた（14.6 vs. 46.7%, p = 0.02）。腋窩リンパ節郭清はMBC症例でより頻度が高かった（32.9 vs. 25.2%, p < 0.001）。エストロゲン受容体（ER）陽性はMBC症例の95.6％、FBC症例の85.3％に認められた（p < 0.001）。HER2陽性率はそれぞれ9.5％、15.7％であった（p < 0.001）。併存疾患はMBCでより多く（57.3 vs. 32.8％）（p＜0.001）。化学療法はMBCでより少なく、内分泌療法の使用はER陽性のMBCとFBCで同様であった。周術期の放射線療法は14.3％、44.3％の症例で実施された。現在、予後のデータを収集し解析を追加中である。

This study was conducted using the National Clinical Database (NCD) to investigate and evaluate the clinicopathological characteristics of male breast cancer. Clinical pathology characteristics of male breast cancer are relevant to the use of the National Clinical Database and the preparation of research protocols for large-scale studies for approval by ethics committees. The NCD registry began in 2012 and ended in 2018. All breast cancer patients were enrolled in the NCD registry. The age, duration, type of operation, type of recipient, type of recipient, HER2 occurrence, family history, comorbidities, systemic therapy, and comparison of male and female breast cancer were included.
