本研究の目的は、成人先天性心疾患患者においてエラストグラフィ法による肝硬度や経頚静脈肝生検による組織学検討などを用いて、先天性心疾患の遠隔期に生じうる、うっ血性肝障害および肝線維化に関して詳細な評価を行い、循環状態指標などの臨床データもあわせて解析することで、成人先天性心疾患における心肝連関の病態について検討することである。大阪大学医学部附属病院に精査加療目的で入院した成人先天性心疾患を対象に、エラストグラフィ法による肝硬度測定、血行動態評価のデータ収集および肝臓組織学的検討を行ってきた。令和3年度から令和4年度にかけて、成人先天性心疾患の中でもとりわけ高い中心静脈圧が持続することによるうっ血性の肝障害が問題となる、Fontan手術後の単心室患者を対象としてデータの解析を行った。具体的にはFontan手術後の成人24例（男性15例、年齢中央値35歳）を対象に、Fontan術後症例における肝硬度と中心静脈圧の関連および画像所見から疑われる肝組織学的変化の有無による肝硬度と中心静脈圧の関連への影響、を検討した。結果としては、Fontan術後患者における肝硬度と中心静脈圧には強い相関関係がみららた。ただし、過去の非先天性心疾患患者における検討結果から算出された、肝硬度から中心静脈圧を推定する予測式にもとづく推定中心静脈圧は実測値より高くなる傾向があった。この傾向は冠組織学変化がある群、ない群ともにみられた。これらの検討結果について、令和5年度に論文投稿予定である。

The tendency of the crown histology is to change the group, the group and the group. The results of this review are scheduled for submission in the fifth year.
