全身麻酔を受ける高齢患者の術後合併症の1つに、術後神経・認知機能障害がある。高齢化が進む現代では全身麻酔下での手術を受ける患者年齢も上がっており、その予防が重要な課題となっている。術後神経・認知機能障害の原因の1つに脳内炎症が考えられている。炎症性サイトカインは人体の様々な内分泌器官から分泌されるが、体重の約40-50%を占める骨格筋も生理活性因子(マイオカイン)を分泌することが分かっている。本研究では、術後神経・認知機能障害の発症とマイオカイン分泌量の関係、および骨格筋量の関係について臨床的見地から検討することを目的としている。予備研究として、75歳以上の予定手術を受けた患者を対象に術後せん妄(postoperative delirium：POD)あり群となし群の2群に振り分け、2群間で術前術後のマイオカイン分泌量及び骨格筋量の関係について検討した。測定するマイオカインは患者血清中のIL-6、IL-10、IL-1β、TNF-α、BDNF、IGF-1、Irisinとし、神経・認知機能の評価にはConfusion Assessment Methodを使用した。観察期間は術後14日目までとし、マイオカインは術前、術後1日目、3日目、7日目、14日目に測定し、骨格筋量は術前、術後7日目、14日目に測定した。PODあり群となし群でいずれの測定ポイントにおいてもマイオカイン分泌量に有意差は認めなかった。また骨格筋量に関しても2群間で有意差はなかった。現在は、引き続き対象患者を75歳以上の予定手術を受ける患者とし、術後神経・認知機能障害の発症とマイオカイン分泌量および骨格筋量の関係について検討中である。

The postoperative complications of patients with general anesthesia and high fever include: 1. postoperative neurological and cognitive impairment. Inflammatory factors are secreted by endocrine organs of the human body and account for about 40-50% of body weight. The purpose of this study is to investigate the relationship between the amount of neuromuscular secretion and the amount of skeletal muscle secretion in the development of postoperative neurocognitive impairment. In this study, the relationship between the amount of serum secretion and the amount of skeletal muscle mass between two groups of patients aged 75 years and older undergoing scheduled surgery was studied. The Confusion Assessment Method was used to measure IL-6, IL-10, IL-1β, TNF-α, BDNF, IGF-1, Irisin, and cognitive function in serum of patients. During the observation period, bone mass was measured before operation, 1 day, 3 days, 7 days and 14 days after operation. The amount of POD secreted is intentionally different from that of the control group. There is a difference between the two groups. The relationship between the amount of secretions and the amount of skeletal muscle mass in patients aged 75 and older undergoing scheduled surgery and the development of postoperative neurological and cognitive impairment is discussed.
