项目摘要

診療参加型臨床実習における重要な学修項目である診療録の記載について継続的に評価する教育プログラムの確立を目指し計画を立案した。医学科5,6年次の選択型臨床実習の中で、学生が作成した診療録に対し、①POMR、SOAPとしての体裁、②病歴聴取、主観的事項の記載状況、③客観的事項の記載状況、④臨床推論の過程、診断のプロセスの記載状況、⑤計画の記載状況、⑥用語（略語）・表現の適切性、⑦倫理的配慮、プロフェッショナリズム、の項目別評価と、概略評価を5段階で行った。6週×4期の実習期間で、各期での中間および終了時の評価のほか、4期を通じた評価の動向について検討を行った。本研究は2020年1月から開始したが、COVID-19の感染拡大に伴う臨床実習の制限のため、途中から患者情報や動画コンテンツの提示によるシナリオ課題に基づいた演習形式での学生診療録を評価対象として研究を継続し、2023年3月末現在、のべ92人の医学生の評価を行った。結果、実習各期における中間時点と比較し終了時の評点が上記全ての項目で有意に上昇し、概略評価も3.2±0.73から4.0±0.63（p<0.01)と有意に増加していた。また全4期の実習期間を通じた評価の変化については、第4期で④⑤の項目別評価および概略評価の有意な上昇を認めた。本研究における複数回のピア・レビュー教育により、医学生の診療録記載技能が向上した。ただ時期別検討で第4期のみ評点の有意な上昇を認めた原因としては、診療録記載に関する既存の学修機会が十分でない可能性、実習の総仕上げや臨床実習後OSCE対策として実習最終期に学修が進んだ可能性が考えられた。アンケートでも本ピア・レビュー教育は概ね好評であった。本研究と同様の形成的評価の普及を促進すべく、今後学会発表や論文報告に向けた準備を進めているところである。
